At a glance
Effects
- Post-cycle testosterone restoration: Blocks estrogen receptors at the hypothalamus/pituitary, lifting negative feedback and stimulating LH/FSH release, which drives endogenous testosterone recovery after anabolic steroid use [5].
- Gynecomastia prevention / suppression: Acts as an estrogen antagonist in breast tissue, blocking the estrogenic signalling responsible for gynecomastia development in men [1][2].
- Estrogen receptor blockade: Active metabolite endoxifen competes with estradiol for ERα and ERβ binding, inhibiting estrogen-driven gene transcription in target tissues [1][3].
- Bone-protective (partial agonist): Exerts partial estrogenic agonism on bone tissue, helping preserve bone mineral density — a clinically useful property in post-menopausal contexts [4].
- Breast cancer risk reduction: FDA-approved to reduce invasive breast cancer incidence in high-risk individuals and as adjuvant therapy following diagnosis of ER+ breast cancer [2][6].

What it is
Tamoxifen Citrate (brand name Nolvadex) is a non-steroidal Selective Estrogen Receptor Modulator (SERM) used clinically to treat and prevent estrogen receptor-positive (ER+) breast cancer, and widely used in performance contexts as a post-cycle therapy (PCT) agent to restore natural testosterone production and manage estrogenic side effects [1][2].
How it works
Tamoxifen is a prodrug: it has relatively low affinity for the estrogen receptor on its own, but is converted in the liver — via CYP2D6 and CYP3A4 — into potent active metabolites (notably 4-hydroxytamoxifen and endoxifen) that bind estrogen receptors with up to 100× greater affinity than the parent compound [1][3]. These metabolites act as estrogen antagonists in breast tissue, blocking estrogen's effects, while acting as partial agonists in bone and the uterus — hence the term "selective" modulator [2][4]. In men and in PCT contexts, blocking estrogen receptors at the hypothalamus and pituitary removes the negative feedback that suppresses gonadotropin release, thereby stimulating the pituitary to secrete LH and FSH and consequently driving testicular testosterone production [5].
Risks & side effects
Most important: The most important serious risk is venous thromboembolism (VTE): tamoxifen therapy is associated with an increased incidence of deep vein thrombosis (DVT) and pulmonary embolism, and is contraindicated in anyone with a history of VTE or who requires concurrent warfarin anticoagulation [7][8].
Common
Serious
- Venous thromboembolism: deep vein thrombosis (DVT) and pulmonary embolism [7][8]
- Stroke [7]
- Uterine / endometrial malignancy (due to partial estrogenic agonism on the uterus) [8]
- Hepatotoxicity and liver abnormalities [7]
- Visual disturbances / retinal changes [11]
- Fetal harm — teratogenic; must not be used in pregnancy [8]
Safety & harm reduction
- History of deep vein thrombosis (DVT) or pulmonary embolism (PE) [7]
- Concurrent coumarin-type anticoagulant therapy (e.g. warfarin) — contraindicated for risk-reduction/DCIS indications [7][8]
- Pregnancy or breastfeeding — tamoxifen is teratogenic and can cause fetal harm [8]
- Known hypersensitivity to tamoxifen or any excipient [7]
- Galactose intolerance (some formulations) [11]
- Liver function tests (LFTs) — hepatotoxicity including cholestasis and hepatocellular injury reported [7]
- Gynaecological assessment: report any abnormal vaginal bleeding promptly — uterine malignancy risk [8]
- Ophthalmological review if visual changes occur — retinal and corneal toxicity possible [11]
- Coagulation status — watch for signs of DVT/PE (leg swelling, chest pain, dyspnoea) [7]
- Bone density (long-term use, especially pre-menopausal users) [4]
- Hormone panel (LH, FSH, total testosterone) in PCT context — to confirm axis recovery
- Warfarin / coumarin anticoagulants: tamoxifen significantly potentiates anticoagulant effect — prothrombin time monitoring mandatory; contraindicated in risk-reduction settings [8]
- Strong CYP2D6 inhibitors (paroxetine, fluoxetine): reduce conversion to active metabolite endoxifen, blunting tamoxifen efficacy [3]
- Weak CYP2D6 inhibitors (venlafaxine, citalopram): minimal effect on endoxifen levels but caution advised [3]
- Aromatase inhibitors (letrozole, anastrozole): tamoxifen reduces letrozole plasma concentrations by ~37% — combination use requires careful consideration [8]
- Cytotoxic chemotherapy: further increases thromboembolic risk when combined [8]
- CYP3A4 inducers/inhibitors may alter tamoxifen and metabolite plasma levels [6]
- PCT context (commonly reported, not a prescription): typically started 1–2 weeks after last anabolic steroid dose, once exogenous androgens have cleared sufficiently to allow HPTA recovery
- Commonly reported PCT dose range: 20–40 mg/day, typically tapered over 4–6 weeks — this is community-reported data only; no official PCT dosing protocol exists [6]
- FDA-approved clinical dosing: 20–40 mg/day in divided doses for up to 5 years (breast cancer setting) [6]
- If taking >20 mg/day, split into morning and evening doses [6]
- Do not use hormonal contraceptives concurrently — use non-hormonal contraception during use and for 2 months after [3]
- Confirm CYP2D6 metaboliser status if efficacy is unexpectedly poor, as poor metabolisers produce less endoxifen [3]
Dosage context
FDA-approved clinical doses are 20–40 mg per day (in one or divided doses), confirmed to reach peak plasma concentration ~5 hours post-dose [6]. Terminal elimination half-life is 5–7 days, with steady-state reached in ~4 weeks [5][6]. In PCT settings, community-reported doses of 20–40 mg/day for 4–6 weeks are commonly cited — these are not prescribed protocols and must be treated as anecdotal ranges only. Under no circumstances should dosage be self-determined without medical supervision.
