At a glance
Effects
- Lean Muscle Growth: Strong upregulation of protein synthesis and nitrogen retention drives meaningful increases in lean muscle mass [3][4].
- Strength Gains: Dose-dependent increases in muscular strength have been observed alongside lean mass accrual [6].
- Bone Density: Historically used to treat osteoporosis; nandrolone stimulates bone-forming cells, supporting skeletal integrity [1][7].
- Red Blood Cell Production: Stimulates erythropoiesis, increasing red blood cell count and oxygen-carrying capacity — a basis for its therapeutic use in anaemia [1][7].
- Nitrogen Retention: Enhanced nitrogen balance promotes an anti-catabolic environment, helping preserve muscle tissue during calorific deficits [3].
- Joint / Connective Tissue Support: Anecdotally associated with collagen synthesis support and reduced joint discomfort, attributed in part to its progestin activity [6].

What it is
Nandrolone Phenylpropionate (NPP) is an injectable anabolic-androgenic steroid (AAS) and ester prodrug of nandrolone (19-nortestosterone), historically used medically to treat breast cancer and osteoporosis in women [1]. It is now primarily encountered in performance and physique contexts, valued for its strong anabolic and comparatively mild androgenic profile [1][2].
How it works
After intramuscular injection, esterase enzymes in the blood gradually cleave the phenylpropionate ester, releasing free nandrolone [3]. Nandrolone then enters cells and binds to androgen receptors (AR) in the cytoplasm; the receptor–ligand complex migrates to the nucleus, where it upregulates genes responsible for protein synthesis, nitrogen retention, and muscle hypertrophy [3][4]. Because the 19th carbon is absent (unlike testosterone), nandrolone is far less susceptible to conversion to dihydrotestosterone (DHT) by 5α-reductase, giving it a higher anabolic-to-androgenic ratio than testosterone [5][6].
Risks & side effects
Most important: The most critical risk is severe suppression of endogenous testosterone production: the body detects exogenous androgens and sharply reduces or halts its own output, which — if not managed with post-cycle therapy (PCT) — can result in prolonged hypogonadism, low libido, and infertility [8][9]. Compounding this, nandrolone use is linked to adverse lipid changes (raised LDL, lowered HDL) and cardiovascular damage that can be irreversible [9][10].
Common
- Suppression of natural testosterone production [8]
- Acne, oily skin, and increased body / facial hair growth [1][11]
- Mood changes: irritability, aggression, or low mood [12]
- Gynecomastia (via partial aromatisation to oestrogen-like compounds) [8][13]
- Injection-site discomfort; requires rotation of sites [11]
Serious
- Adverse lipid profile: decreased HDL ('good') cholesterol, increased LDL ('bad') cholesterol, raising atherosclerosis risk [9][10]
- Cardiovascular strain: elevated blood pressure; oedema with or without congestive heart failure reported [10][14]
- Prolonged hypogonadism / infertility if PCT is omitted after cessation [8]
- Virilisation in women: voice deepening, clitoral enlargement (may be irreversible), menstrual disruption [14][10]
- Oligospermia and reduced ejaculatory volume with chronic use [14]
- Hepatic strain at high or prolonged doses; liver cell tumours reported with anabolic steroid therapy [9][14]
Safety & harm reduction
- Pregnancy or breastfeeding — Pregnancy Category X; causes fetal harm [14]
- Males with known or suspected prostate carcinoma or breast carcinoma [14]
- Females with breast carcinoma and hypercalcaemia (stimulates osteolytic bone resorption) [14]
- Pre-existing significant cardiovascular disease or severe hypertension [8]
- Pre-existing liver disease [8]
- Paediatric / adolescent patients (risk of premature epiphyseal closure) [1]
- Lipid panel (LDL / HDL) — at baseline and every 6–8 weeks during use [9][10]
- Liver function tests (ALT, AST, bilirubin) — particularly at higher doses or prolonged use [8][9]
- Haematocrit / haemoglobin — erythropoiesis stimulation can raise blood viscosity [7]
- Blood pressure — monitored regularly throughout a cycle [10]
- Serum testosterone and LH/FSH — to gauge suppression and guide PCT timing [8]
- PSA in older males [14]
- Anticoagulants (e.g. warfarin): NPP enhances anticoagulant effect — increased bleeding risk; close INR monitoring required [13]
- Insulin and oral hypoglycaemic agents: NPP may alter blood glucose, requiring dose adjustment [13]
- Anabolic steroid therapy may decrease thyroxine-binding globulin, altering thyroid function test results [14]
- Other hepatotoxic drugs: additive liver stress [8]
- Other androgens / AAS: additive suppression and cardiovascular burden
- Post-Cycle Therapy (PCT) is essential after any nandrolone cycle to restore endogenous testosterone production — commonly employing SERMs (e.g. clomiphene, tamoxifen) for several weeks [8][13]
- Due to the shorter ester, PCT can typically begin sooner than with nandrolone decanoate — allow ~1 week after last injection before starting PCT [8]
- Aromatase inhibitors (e.g. anastrozole) may be used during cycle to mitigate oestrogenic/progestin side effects such as gynecomastia [8]
- Run a testosterone base alongside NPP to offset suppression-related low-androgen symptoms during cycle [6]
- Cardioprotective lifestyle measures (omega-3 supplementation, aerobic conditioning, dietary cholesterol management) are warranted throughout [10]
Dosage context
Commonly reported performance doses range from approximately 200–600 mg per week administered in split injections (2–3× weekly) to maintain stable plasma levels given the shorter ester [6]. These are non-prescribed, community-reported figures only — not a medical recommendation. Higher doses markedly increase side-effect risk, and no dose is free of suppression or cardiovascular impact [6][8]. Individual response varies; conservative approaches start at the lower end of this range.
