What it is
Methandienone (metandienone, sold as Dianabol) is an orally active anabolic-androgenic steroid (AAS) derived from testosterone, mostly no longer prescribed medically and now used non-medically for physique and performance enhancement.[1][3] It is said to be one of the most widely used AAS for such purposes historically and today.[1]
How it works
Methandienone binds to and activates the androgen receptor, producing strong anabolic and moderate androgenic effects.[1][7] This drives dramatic increases in protein synthesis, nitrogen retention, glycogenolysis and muscle strength over a short period.[1][2] It is 17α-alkylated so it survives oral first-pass metabolism through the liver, which is also why it stresses the liver, and it is a substrate for aromatase so it converts to estrogen.[2][7]
Risks & side effects
Most important: As a 17α-alkylated oral steroid, methandienone is hepatotoxic: prolonged use or high doses can cause cholestasis, elevated liver enzymes, hepatic adenomas and, in extreme cases, liver cancer.[3][5] Liver injury can occur without symptoms early on, so it must not be used without liver-function monitoring and avoidance of alcohol and other liver-stressing substances.[6][3]

- Androgenic effects: acne, oily skin, and scalp hair loss in predisposed individuals.[1][2]

- Premature cardiovascular disease and prostatic hypertrophy in men with chronic use.[6]
