What it is
Erythropoietin (EPO) is a glycoprotein hormone produced by the kidneys, and recombinant versions (epoetin alfa, darbepoetin alfa) are erythropoiesis-stimulating agents (ESAs) used to treat anemia caused by chronic kidney disease, chemotherapy, and certain other conditions[1][2]. It works by stimulating the bone marrow to make more red blood cells[5].
How it works
EPO binds to the erythropoietin receptor (EpoR) on red-cell progenitor cells in the bone marrow and activates a JAK2 signaling cascade, driving those cells to divide and mature into red blood cells[1][6]. Natural EPO is released by the kidneys in response to low blood-oxygen levels; recombinant ESAs reproduce this signal pharmacologically[2][5]. By raising red-cell numbers and hemoglobin, EPO increases the blood's oxygen-carrying capacity[6].
Risks & side effects
Most important: EPO carries an FDA boxed warning: it increases the risk of death, heart attack, stroke, venous thromboembolism (blood clots), vascular-access thrombosis and tumor progression — risks that rise when hemoglobin is pushed above roughly 11 g/dL, and no dose or target has been found that eliminates them[9][11]. Misuse in healthy or non-anemic people to thicken the blood is especially dangerous because it raises blood viscosity and clotting/stroke risk[6][10].

Safety & harm reduction
- People with uncontrolled hypertension[3]
- Anyone with a history of pure red cell aplasia (PRCA) following EPO or other erythropoietin proteins[3]
- Anyone with a history of serious allergic reaction to epoetin products[3]
- Pregnant or breastfeeding women, neonates and infants should not receive multi-dose vials containing benzyl alcohol[3]
- Cancer patients where the chemotherapy is intended to cure should not receive ESAs[10]
- Hemoglobin: monitor every 1-2 weeks after starting and adjust dose; reduce or interrupt if hemoglobin approaches/exceeds 11-12 g/dL[2][13]
- Blood pressure: monitor regularly (including at home) and reduce dose if hypertension cannot be controlled[8][10]
- Iron status should be assessed to support red-cell production (standard ESA practice)
- Use the lowest dose needed to avoid transfusions; do not target a hemoglobin above ~11 g/dL[11]
- Pair with adequate iron; reduce/withhold dose if hemoglobin rises faster than ~1 g/dL in 2 weeks[13]
- Discontinue ESAs after completing a chemotherapy course[10]
- Avoid use in healthy/non-anemic individuals for performance — clotting and cardiovascular risk rises with no anemia benefit[6][10]
Dosage context
Commonly-reported clinical context (FDA labeling, NOT a prescription): for anemia of chronic kidney disease, a typical adult starting dose is 50-100 units/kg given IV or subcutaneously three times weekly, initiated only when hemoglobin is below 10 g/dL, with the dose reduced or withheld as hemoglobin approaches 11-12 g/dL[12][13]. Lower target hemoglobins are safer; higher targets and higher doses increase the risk of death and clots[9][11]. These are physician-supervised regimens with regular bloodwork — self-dosing is dangerous.
