What it is
How it works
It antagonises estrogen receptors in the hypothalamus and pituitary, blocking estrogen's negative feedback on the hypothalamic-pituitary-gonadal axis [4][5]. This raises GnRH, LH and FSH secretion, which stimulates the testes' Leydig cells to make more testosterone while FSH supports sperm production [2][3][5]. Because it restores the body's own testosterone rather than replacing it, it does not suppress spermatogenesis the way exogenous testosterone (TRT) does [1][6].
Risks & side effects
Most important: As with other SERMs, enclomiphene carries a small but real risk of venous thromboembolism (deep vein thrombosis or pulmonary embolism); absolute rates in trials were under ~0.5%, but risk rises in smokers and people with clotting disorders or a clot history [7][8][9]. Sudden calf pain, one-sided leg swelling, chest pain or shortness of breath means stop the drug and seek same-day medical care [8].
