What it is
Telmisartan is a prescription angiotensin II receptor blocker (ARB) used to treat hypertension and to reduce the risk of cardiovascular events such as stroke and heart attack [1][2]. It is one of the most pharmacologically distinct ARBs, distinguished by the longest plasma half-life, highest lipophilicity, and strongest AT1 receptor binding affinity in its class [3].
How it works
Telmisartan selectively blocks angiotensin II type 1 (AT1) receptors, preventing angiotensin II from constricting blood vessels and stimulating aldosterone release — both of which raise blood pressure [1]. This causes vasodilation and reduces sodium/water retention, lowering blood pressure [1]. Uniquely among ARBs, telmisartan also acts as a partial agonist of PPARγ (peroxisome proliferator-activated receptor gamma), which contributes additional anti-inflammatory, insulin-sensitising, and visceral-fat-reducing effects beyond its blood-pressure action [2][3].
Risks & side effects
