What it is
Clomiphene citrate is a non-steroidal selective estrogen receptor modulator (SERM), classed by MedlinePlus as an "ovulatory stimulant," that is FDA-approved to induce ovulation in women with anovulatory infertility [1][6]. It is also widely used off-label in men to raise endogenous testosterone and preserve fertility [4][9].
How it works
Clomiphene binds estrogen receptors in the hypothalamus and occupies them for longer than estrogen, interfering with receptor recycling and blunting estrogen's normal negative-feedback signal [3][7]. The brain "reads" estrogen as low, so it increases pulsatile GnRH, which drives the pituitary to release more FSH and LH [1][3]. In women this matures an ovarian follicle and triggers ovulation; in men the extra LH stimulates the testes' Leydig cells to make more testosterone while preserving sperm production [1][4][8]. It is a racemic mixture of two isomers — enclomiphene (anti-estrogenic, short-acting) and zuclomiphene (estrogenic, long-acting) [5][7].
Risks & side effects
Most important: The most important risks are visual disturbances (blurring, spots/flashes, scotomata) — which the FDA label warns may, in some cases, be prolonged and possibly irreversible — and, in women, ovarian hyperstimulation syndrome (OHSS), which can progress rapidly and, in severe cases, has caused death from hypovolemic shock or thromboembolism [5][7]. Any visual symptoms mean you should stop and get an urgent eye exam [5].

Dosage context
Commonly-reported, NOT a prescription. For ovulation induction the FDA-labelled regimen is 50 mg once daily for 5 days, beginning around day 5 of the cycle, increased to 100 mg/day in later cycles only if ovulation does not occur, with the lowest effective dose used and a maximum of 100 mg/day for up to 6 cycles [1][6]. In off-label male hypogonadism studies, lower doses such as 25 mg daily or 25 mg every other day have been reported [9][10].
