What it is
Clenbuterol is a potent, long-acting beta-2 adrenergic agonist (a sympathomimetic amine) developed as a bronchodilator for asthma and COPD in some countries outside the US.[1][2] It is not FDA-approved for human use anywhere in the United States and is widely misused off-label for fat loss and muscle preservation.[5][6]
How it works
It binds to and activates beta-2 adrenergic receptors found in airway smooth muscle, skeletal muscle and fat tissue, mimicking the action of adrenaline.[1][3] This relaxes the airways (bronchodilation) and, by raising basal metabolic rate, drives thermogenesis and fat oxidation; in humans it increases energy expenditure and fat oxidation, though its purported muscle-building effects are largely demonstrated only in animals.[1][4][5] The downstream mechanisms behind its lean-mass and fat-loss effects remain unclear.[3]
Risks & side effects
Most important: Clenbuterol's biggest danger is cardiac toxicity: even low doses can trigger severe tachycardia, dangerous drops in blood potassium (hypokalemia), arrhythmias, myocardial injury/heart attack and, rarely, cardiac arrest and death — and there is no antidote.[7][8][9] Its long half-life means toxic symptoms can build over days, and outcomes are unpredictable: identical doses can leave one person with mild tremor and another in the ER.[10][11]

Safety & harm reduction
- Anyone with existing cardiovascular disease, hypertension, arrhythmia or cardiac hypertrophy — clenbuterol directly stresses the heart and can cause myocardial injury.[7][10][13]
- Athletes subject to drug testing — it is prohibited by WADA at all times with no permitted threshold.[14][15]
- Anyone using it without medical supervision; it is not FDA-approved for human use and toxicity occurs even at low doses.[5][7]
- Other stimulants such as caffeine compound blood-pressure and cardiovascular strain.[8]
- Drugs or conditions that lower potassium magnify arrhythmia risk.[7]
- Beta-blockers (e.g. metoprolol) are used clinically to counteract clenbuterol's beta-agonist effect in overdose — meaning concurrent stimulant use works directly against them.[12][16]
- There is no reversal agent or antidote; overdose management is supportive — IV fluids, potassium/electrolyte repletion, benzodiazepines for agitation, and beta-blockers for tachycardia.[12][16][9]
- Because of the long half-life, toxic effects accumulate and can persist 1–8 days; do not 'top up' if effects feel weak.[5][11]
- Seek emergency care for chest pain, fainting, severe palpitations or breathing difficulty.[7][9]
Dosage context
There is no approved human dose; because clenbuterol is not FDA-approved for people, no standard dosing recommendation exists.[5] Reported athletic misuse is commonly in the 50–100 µg range or higher, and toxicity has been documented at doses as low as 20 µg, with effects over roughly 120 µg increasing tremor, headache and dizziness.[17][7][2] These are commonly-reported figures for context only — not a prescription or endorsement; the drug is dangerous and unpredictable even at low doses.[10][7]
