What it is
Boldenone undecylenate (brand names Equipoise, Parenabol) is an injectable anabolic-androgenic steroid (AAS) used mainly in veterinary medicine, especially in horses, and is no longer approved for human use.[1][3] It is the long-chain ester of boldenone, a testosterone derivative, and is used illicitly to improve physique and performance.[1][2]
How it works
Boldenone undecylenate is a prodrug: after intramuscular injection it forms an oil depot from which it is slowly released and hydrolysed to active boldenone, which acts as an agonist of the androgen receptor.[1][4] Boldenone increases nitrogen retention, protein synthesis, appetite, and stimulates erythropoietin release from the kidneys, driving red-blood-cell production.[5] It has strong anabolic but only moderate androgenic effects, with low estrogenic activity and little progestogenic activity.[1] Because it is not 17α-alkylated, it carries little or no direct hepatotoxicity risk of the kind seen with oral steroids.[1]

Most important: The most important risk is a marked rise in red blood cells and haematocrit (polycythemia) — boldenone is among the most notorious AAS for this — which thickens the blood and raises the risk of clots, stroke, and heart attack.[7][8] Combined with unfavourable shifts in cholesterol and its very long clearance, cardiovascular monitoring is essential.[8][9]
