What it is
Exemestane (brand name Aromasin) is a steroidal aromatase inhibitor — a hormone therapy, NOT an anabolic steroid as the vendor spec labels it.[1][3] It is approved to treat estrogen-receptor-positive breast cancer in postmenopausal women, and in performance contexts is used off-label by men to control estrogen during anabolic steroid cycles and in post-cycle therapy.[3][11]
How it works
It is structurally related to androstenedione and acts as a "false substrate" for the aromatase enzyme: the enzyme processes it and becomes permanently, irreversibly bound and inactivated — an effect called "suicide inhibition."[5][2] By disabling aromatase, it blocks the conversion of androgens (such as testosterone) into estrogens, lowering circulating estradiol and estrone.[5][1] Because the enzyme is destroyed rather than merely blocked, the body must synthesise new aromatase to recover, so there is little estrogen "rebound" after stopping.[8]
Risks & side effects
Most important: Because exemestane sharply and durably lowers estrogen, the most important risk is estrogen deprivation: accelerated bone-mineral-density loss raising osteoporosis and fracture risk with longer use, and — if estrogen is crashed too low — joint pain, low libido, fatigue and impaired wellbeing.[5][8] Self-dosing without estradiol bloodwork can easily over-suppress estrogen, so any use should be guided by sensitive E2 testing, not symptoms alone.[10]

Safety & harm reduction
Dosage context
Clinically the established dose is 25 mg once daily, taken after food — this is the lowest dose giving maximal estrogen suppression in breast-cancer treatment.[6][9] In off-label male estrogen-management, community sources commonly report much lower amounts such as 12.5 mg every other day, titrated to sensitive estradiol bloodwork (a frequently cited target of ~20–40 pg/mL).[10][11] These are commonly-reported figures, NOT a prescription; over-suppressing estrogen is harmful, so any use should be guided by E2 testing.[10]
