What it is
Vitamin E is a group of eight fat-soluble plant-derived compounds — four tocopherols and four tocotrienols — of which alpha-tocopherol (α-tocopherol) is the most biologically active form retained in human tissues [1]. It is an essential micronutrient used clinically to treat deficiency states and studied widely for its antioxidant, anti-inflammatory, and neuroprotective properties [2].
How it works
Alpha-tocopherol donates a hydrogen atom from its phenolic hydroxyl group to neutralise lipid peroxyl radicals, interrupting the chain reaction of lipid peroxidation in cell membranes [3]. In doing so it becomes a relatively unreactive tocopheroxyl radical, thereby protecting polyunsaturated fatty acids and membrane integrity [3]. Beyond direct radical scavenging, tocopherols and tocotrienols can also act as signalling molecules, activating kinases and transcription factors to alter gene expression via antioxidant-independent pathways [4]. Vitamin E is fat-soluble and absorbed via the lymphatic system alongside dietary fats, meaning absorption is significantly impaired in conditions of fat malabsorption [5].
Risks & side effects
Most important: At high supplemental doses, Vitamin E has clinically significant anticoagulant and antiplatelet effects — it reduces glutamate needed for clotting factor IX production and enhances the action of warfarin, increasing the risk of serious bleeding including gastrointestinal bleeds and intracranial haemorrhage [7]. This risk is especially dangerous in anyone already taking anticoagulants or antiplatelet drugs [6].
